Mike Hebert is the Senior Director UM of Care Management & Geriatrics at Reliant Medical Group. Mike holds three advanced degrees in nursing, business, and social work. Mike Hebert joined Reliant Medical Group in 2007, bringing many years of direct care experience within the healthcare industry. Prior to joining Reliant, Mike held several positions for both Davita as well as Montachusett Home Care Corporation. Mike’s experience includes program development, operational design/ integration and day-to-day business operations. During the span of Mike’s career, he has worked on a variety of projects through various settings including primary care, mental health and specialty care management. Mike has played a key role in many key strategic organizational objects including implementation of infrastructure necessary to take on full risk and UM/CM health plan delegation.
