Requesting Accommodations


Students with learning disabilities, ADHD/ADD, chronic medical conditions, physical disabilities, or psychiatric disabilities who plan to request academic accommodations must self-identify and provide appropriate documentation of disability to the Director of Disability Services.

Individual Education Plans (IEP) and 504 Plans are not considered documentation and do not carry over to higher education.

If you would like to request disability accommodations, please fill out the 2007 Accommodations Consent Release form and return it to Director of Disability Services, Sharon de Klerk, in the Academic Support Center on the 2nd floor of the d'Alzon Library. This form can also be e-mailed to you by Sharon, and hard copies are available in her office.
